Essential Functions & Responsibilities
Individual will be responsible for providing a wide variety of services and will work with a multi-disciplinary team of Case Management staff, physicians, nursing staff, social service agencies and others to meet the needs of a diverse patient population including inpatient, outpatient and palliative care. Duties will include providing psychosocial assessments, crisis and short-term counseling; community resource education; and discharge planning for patients/families in both the inpatient and outpatient settings. Will also assess psychosocial needs of patients and families; educate patients and families about available options for care; and assist patients/families with decision making and linking them with appropriate sources of help.
Qualifications/Experience
Must have a Master's Degree in Social Work from a school accredited by the Council on Social Work Education. Must also have a license to practice Social Work in Iowa, LISW preferred, or be eligible for licensure. In addition a minimum of 2 years of medical social work experience is strongly preferred.
